We will not share your information with any third party outside of our organization, other than as necessary to administer the study.

Who will have access to information about you?

By entering your information on this website and through attached questionnaires, you are allowing the study team to have access to your personal health information required for participation in the study. The research team includes the Principal Investigator (study doctor) and other personnel involved in this specific study. Your PHI will be used only for the purpose(s) described in the section 'What is the reason for doing this research study?'.

The Institutional Review Board (IRB) and accrediting agencies may inspect and copy your records, which may have your name on them. Therefore, total confidentiality cannot be guaranteed. If the study results are presented at meetings or printed in publications, your name will not be used.

You need to know that laws protect your health information when it is held by hospitals and healthcare providers. But if your health information goes to someone else, your health information may not be protected by those laws. They may share your information with others not listed here.

Your Access to and Control Over Information 

You may cancel your authorization for further collection of PHI for use in this research at any time by contacting the Principal Investigator in writing: Peter Swann, MD, Principal Investigator, 2165 Toyon Drive, Healdsburg, CA 95448. However, the PHI which is included in the research data obtained to date may still be used. If you cancel this authorization, you will no longer be able to participate in this research.
